why will EU market be different? I.e. why would EU market have a fast ramp and peak out WAY above ~ 100k scripts?

I do not know it will have a fast ramp or not but EU is different.

"Everybody" are insured, out-of-pocket cost is low. Once you have an agreed national reimbursement in place it depends on how fast and well could you teach / inform prescribers, it does not depends on patients' decision (I mean: "Could I afford it or not?").

e.g.:
(i) Finland: In 2021, the annual maximum was set at $620. If the patient reached it, the patient become entitled to an additional reimbursement for the medicine costs. That means that the patient only paid a $3 copayment for the medicines for the rest of the year. = out-of-pocket: $54 / script

(ii) Sweden: Patient has to pay:
$ 0 - 117: 100%
$ 117 - 171: 50%
$ 171 - 219: 25%
$ 219 - 235: 10%
When you have paid $235 you do not pay more for medications that are included in the high-cost benefit scheme during the remainder of the high-cost protection period. = out-of-pocket: $19.5 / script

(iii) Germany: copayment ranges from a minimum of $6 to a maximum of EUR $11 per prescription
