Youre correct, simply optimizing/titrating doses is not precision medicine.

However, identifying the many variables which influence efficacy of the drug, and then using that information to assist in optimizing efficacy, imo is precisely precision medicine. For example, how ones genetic makeup and/or gut microbiome influence drug efficacy, both positively and negatively, as Boi put it, is Precision Research (medicine). Utilizing these findings to optimize efficacy is hopefully where all this extensive and time consuming research is taking us.
