If you bought that stock before 2011 you might have a problem tracking it down. If it was purchased before that year, your broker does NOT have to provide you or the irs with cost basis. That would be your responsibility. If it was purchased prior I would advise trying to get them to find the name of the company and if they can you might find some old confirmations lying around your house somewhere. If not, the worst that can happen is you ask them to remove the cusip from your account and declare it worthless, but you don't get to use the loss because you don't know what your cost basis is.
