A few years ago, I submitted a manuscript to a medical journal and was asked to make some small revisions by the reviewers. We made the amendments quickly and the paper was accepted for publication within 5 weeks of submission. It appeared as an early on-line paper on the journal's website 8 weeks later.
The process can be fairly rapid if the story is good and the journal wants to publish.
