I don't think you can conclude that there will be spinal taps in a trial whose protocol is certainly not yet established. I expect they will administer cognitive tests, etc., in the course of diagnosis. I also expect ordinary blood samples may be drawn to see if people are taking their pills. But I don't see a need for spinal taps that are expensive, painful, and carry additional medical risk -- and a deterrent to recruitment.

I am not sure I understand your "exclusion" question. But I will try this answer: You have to fish where the fish are. For a COVID vaccination trial, that means recruitment in areas of active spread; for an AD prevention trial, it means the recruitment of a healthy but at-risk population, i.e., the elderly. I would expect AVXL to note genetic APOE status but not restrict participation based on that criterion.
