ER visits are almost higher than ever despite the opening of quick cares that I understand and loth. A competent urgent care is awesome. That has not been the norm. And the free standing ER’s are blossoming once again. Each with probably 20-30 more visits than they need to be profitable. All corporate owned. But not corporate reimbursed.

But volume in most true ER’s in vegas is actually now higher than pre covid. We have had a population boom. And a major anxiety boom. and a major decrease in provider competency. The stuff the urgent cares send us is insane. You can google the competency rate of unsupervised nurse practitioners and you will soon see the problem in the urgent care or emergency(unsupervised) setting……I work with very trusted physician assistants and nurse practitioners. I love them…and trust em. I’m no god…but damn if I did not train for an ungodly number of hours..absorbing all the time. And the new physicians are woefully ill prepared. Most are competent. All are undertrained. There might be up to 20% that are honestly not ready….but the zprograms and companies keep pumping em out regardless. My replacements I fear. For to question is to mark yourself.

The problem is we have a puppy mill of residents…nurse practitioners and physician assistants right now. I wrote a letter 3 years ago for a hard working student…but honestly…I probably should not have. She just got in…my complacency and not wanting confrontation is part to blame. Honestly I should have told her to pursue something else…but it was her dream and she was “close”. So I tried to tip her over the edge…the only good thing is she is probably a better person than me….however…a nice ER doc is not necessarily the best…I’m a mix of both…50 years old…burning man. Father. Spent a night in jail…

and the scary thing is…with the residency hours requirements in place now…new physicians about to be attendings have probably had 35%less patient contacts than I did over my residency. I hear their bitching every day. The changes of a few year’s ago….

Was I bullied? Yes…was it necessary? I honestly say no…it was not…but tearing into some one…sometime it’s needed…..I needed it.

And god forbid ya lay into one of these snowflakes …..they suck!!!!! But it’s your job they will take with HR blessing. It ends in your dismissal. I’m not kidding here. I sold my practice…the largest in Nevada….6 years ago. I work cause life is expensive and my kids still in school. And I like being an ER doctor…or at least I use to…..Also I still work cause AMRN has treated me badly. Lesson learned. I fear for our future care. Only 80% of my current residents do I believe in. That’s diff than a monthly grade. I’m saying 20% of these people are nice…unstressed…(they have no idea) but entirely inadequate….and these are the ED grads. The Internal Med grads are even more dispersed and inadequate. I made lots of noise….but I’m now done. Deaf ears
