Here's what I typically do. Each AM I typically scan the top gainers list over looking at the boards. "Tools" tab on the top of the page, then on the drop down "Data tools" and on the third drop down "Top gainers". Then on the gainers page for all markets, along the top edge look for "US OTC" and you can look at the top gainers and losers, volume etc....

On TAWNF, it was up multi hundred % at .90 when I spotted it. Typically I would've shy'ed away but saw on twitter references of TAWNF making a move like NOKPF did. I checked NOKPF and saw it started just like TAWNF was doing and that one hit $13 yesterday AM so I decided to buy at .90. It then dipped to .71 ask for a few minutes and I though.... NO!!!!!!!!!! And then it started up and never looked back.

So it's a combination of the top gainers list, seeing what's being tweeted on the ticker, looking at L2 and then seeing how many shares are outstanding. In this case, TAWNF was up several hundred % on 74k volume at the time and that to signaled me to buy as it appeared there weren't many shares here despite Etrade showing 3.2 billion OS

So, the answer to your question; a combination of the above. #1 though is the top gainers list to look for them.
