I posted the link. Post #329628. Bottom of page 49 you will see coverage for gl and Vascepa. The date on the formulary is 3/1/2021. It is possible it is updated quarterly.

I mean the one that is 1/1/2021 and includes gV (that you saw).
How could it (gV) be on the list if it is updated quarterly? (01/01 - 04/01 ... so on)
