"Effective May 1, 2021, we'll exclude brand and generic vascepa (Icosapent Ethyl) from benefit coverage when prescribed to treat severe hypertriglyceridemia (greater than or equal to 500 mg/dL). Vascepa, when prescribed as an adjunct to maximally tolerated statin therapy for cardiovascular risk reduction in adult patients, may continue to be covered once prior auth criteria is met."
