INTRODUCTION:
Although rates of episiotomy are declining, post-episiotomy pain remains a complication in obstetric care. Current management includes NSAIDs and Benzocaine, but the use of Benzocaine has been linked to iatrogenic methemoglobinemia, with greatest risk in newborns. We hypothesized that a novel drug formulation, currently being investigated in diabetic neuropathy, can be used to attenuate post-episiotomy pain.
METHODS:
This pilot study was conducted in an outpatient obstetrics clinic on postpartum women aged between 18 and 39, who underwent vaginal delivery with episiotomy. Spray bottles with the investigational product were distributed to the patients to use without regulation. A survey to assess post-episiotomy pain using the Visual Analog Scale (VAS) was administered to the patients. Pain attenuation, time to pain attenuation, and quality of pain were measured.
RESULTS:
The cohort consisted of 25 women (including 5 non-responders). In responders, pain was attenuated by 4.0 scale units. Average time to analgesia was 9.25. Average pain ratings after treatment were compared to average pain ratings before treatment using a unidirectional paired sample t test (df=24). The average pain ratings for the entire cohort after administration of Relivatrol© were significantly lower than average pain rating prior to administration t (24)=7.41, P<.0001.
CONCLUSION:
We concluded that Relivatrol© significantly decreases post-episiotomy pain. Being a pilot study had multiple limitations; we developed a protocol for a larger investigation and are currently collecting data involving a greater number of subjects, a control arm, pharmacokinetic parameters, and an analysis of quality of life impact.

Today we’d like to introduce you to Andrew Damian Forbes.

Andrew Damian, let’s start with your story. We’d love to hear how you got started and how the journey has been so far.

I was born in Mandeville Jamaica. Raised in New York City. After graduating from SUNY Albany with a FinTech dual degree, I worked for a WallStreet based asset management company called First Investors for almost a decade. The training and mentorship I had with INROADS.org plus an internship with MetLife prepared me for this stage. Had much success with them, some of the experiences I had with that company, awards won, people I met, etc. will live with me forever. Loved the company but after it got sold it lost the family feel which I favored so much so I had the itch to make a change. Relocated to Miami Beach after the hurricanes in NYC. Irene chased me home from the Turks & Caicos for a second experience, then later hurricane Sandy came, this was the last straw. Developed and funded my first true startup business JozuForWomen.com while working in this firm, this spark just never stopped. I was always entrepreneurial developing solutions for problems both small and large since I was very young, I wrote more about this in my book, this was probably always the path for me, one of a trailblazer. I love to emulate people but I am not comfortable imitating. I love connecting people, many friends consider me a missing link when they need to solve a situation that seems like a hail mary. I love that term. ‘Hail Mary’, for me, it represents a salute to someone I hail almost daily, my amazing mother Mary Burton Forbes. My father, Novello Fernando Forbes, is fantastic as well. They, as well as my son Leo, are truly my inspiration and the big reason I am filled with a feeling of such gratitude and it fuels me eternally.

Has it been a smooth road?
If it was a smooth road, we would never ‘find’ gems, would we? For they must deal with the pressure which makes them smooth and also priceless. I have made mistakes but they are the reason I learn. Much like when we proceed to walk, we fall as the saying goes seven times but we get up eight. We have heard this said over and over again but to live it is to know it, smooth seas never made a great sailor.

In my experience building startups, you meet people who have masks or rather egos but they always show up in times of trial so you must trust your instincts. For knowledge isn’t free, you have to pay attention. But what comes out of it, is pure beauty. You develop resilience like no other.

For the first decade of my career, I was driving the success but I could not reach as many people as I hoped with a small team, massive collaboration was needed. When you start to collaborate you must find a delicate balance within your team and it is easy to keep when you are “winning” but much more difficult when the tide turns much like in sports. If we check the egos at the door and decide that team-work makes the dream work then many hands will always ultimately make light work.

Please tell us more about your work, what you are currently focused on and most proud of.
I love building supportive networks. Much like ecosystems, they must be symbiotic. In other words, people or organisms helping and supporting each other. I believe what sets me apart is I truly love to work for great causes and I don’t do it for the applause or rewards that come with the work. The work itself is fulfilling enough as long as it’s helpful.

I serve as an ambassador and lead volunteer for The Little Lighthouse Foundation (LLF), thelittlelighthouse.org as well as Les Couleurs, lescouleurscharity.org. Both have fancy galas and donor receptions etc, yet the Toy Drive and Halloween Party with the children for the LLF and the surprise handmade thank you collages I receive from the children of Les Couleurs in Haiti just make the effort and hail mary’s for these organizations all worth it in the end. The LLF Team of Priscilla Roman, Jonathan Babicka, Aaron Resnick, Charlie Venturi, Rob Sena as well as Aura Copeland plus the Les Couleurs Team, all work tirelessly in the communities their organizations support so for me, it is a blessing to be able to do my part to enhance their collective missions in any way I can.

Also, the impact of social media to help these causes is astonishing. I have a helped these causes further their reach using social media effectively to spread their messages to friends and family far and wide. This could only happen through massive collaboration. A friend of mine developed machine learning technology called MySocialGrow, mysocialgrow.com, and approached me about working together and it helped my brand as well as many of the same people and organizations I collaborate with to increase their brand exposure. As a friend of the New World Symphony, nws.edu, we always have a massive push for Pulse to make the experience more social. eMerge, emergeamericas.com, Unbound, unbound.live, and ambassador work with Miami VR Expo, miamivrexpo.com, have led to great connections like my dear friends Brittany Jarrin and Jossua Parini at CreativeGLUE, thecreativeglue.com. My partner Joseph Patterson and I represent MediaRamp, mediaramp.io, at most of these events or other startups we collaborate with and support.

Now, with an audience, we can build a launchpad for products and experience that can truly benefit people locally and globally. All through working together. Is there something we can do as a city to improve the outlook for businesses like yours?
Miami is a magical place, it is perfect for social entrepreneurs. We get more visits than most places because of our massive port, cruise industry, conferences, festivals including music, culture, the arts, the list goes on and on.

If only for the year-round sunshine and no state tax, I would say give Miami a shot. You will meet many people, experience much and worst case you, if done right and you still have to return to where you came from, and that’s a big if, you will have become much cooler in the process.

There is always room for improvement but once again, if all the stakeholders make it, our collective mission to meet and work with each other everyone who comes may just never leave.
