The best is yet to come: "When PD-1 mAb blockade was administered concomitantly
with DC vaccination, a highly significant survival benefit resulted.
Approximately 40% long-term survival was observed selectively in the
combination treatment group (Figure 2D). As we described before, there was
no therapeutic benefit with DC vaccination alone in these large, established tumors.
Similarly, there was no survival benefit noted with PD-1 mAb blockade alone."

How DCVax Direct Works (Doc logic)

Uninhibited Circulating DCs will pick up exozomes in the blood stream or early while still exposed at a landing site.
Exozomes use a mesenchymal trait for mobility and DCs work well against this.
Tumors caused by cancer produce inhibitory signaling that keeps DCs from doing their work.
In the blood stream uninhibited circulating DCs,which are in low numbers,
can be overwhelmed by the number of exozomes released which allows some cancer cells to set up camp in a new location.
The introduction of large numbers of DCs into the blood stream at an early enough stage and in an
uninhibited state would allow certain patients to benefit substantially.
DCVax-L is already matured so it mostly would not be picking up more antigen in most cases but
there is natural heterogeneity in the L population of cells along with tumor specific antigen to be
presented to T-cells and others and this and should be able to help a greater patient population.

How are DCs activated?
Dendritic cell activation can occur in two ways:

(1) Directly by conserved pathogen molecules.

(2) Indirectly by inflammatory mediators (which are produced by other cell types that recognize such molecules),
cellular stress molecules, or disturbances in the internal body environment.

It is possible that these different activation pathways have evolved to ensure the early detection of infections
before invading pathogens replicate in overwhelming numbers.
