Could it be he is toying with people, and will later say, look, if you listened, I meant about 100 out of the 500 total we were tracking. In which case he is actually saying about 20% survival at respective five years for all 500 patients tracked?

I don't think Les is stating survival data is based on the top 100 of 500. Listen to what he said starting at the 28:40 mark. He is saying the top 100 on a blinded basis are living to about 5 years. The only patients blinded are the 331 in the phase 3 trial. The other patients under expanded access protocol and the information arm were never blinded. I suppose it would have been more accurate to say the top 100 of the 331 in the trial are living with a mOS of almost 5 years (58.4 months). But to me, the top 100 "living to about 5 years" taken in context is the same as saying "at about 5 years half would have died and half would be still living".
