It's very simple. All that needs to be done is for someone to call an MM, saying he wants to trade the stock, and asking him to apply to OTC Markets to publish unsolicited quotations. This is the form the MM fills out:

Trouble is, they used to discourage MMs from doing that, except in unusual circumstances. But late last year, they changed their mind. The result has been a mess.
