personally I'd go with an undiagnosed congenital heart defect, such as a single ventricle defect. Kid is 10, probably never been on an airplane before, nor high altitudes (airplane cabin is pressurized to the equivalent of ~6,000' in elevation). Kid doesn't really feel it until they're up in the air for a while, change in pressure results in stress / triggers something. Parents aren't sure what to do, think maybe it's nerves / anxiety re: flying or something else, decision to turn back is delayed. Things get worse, flight is turned around, but by then it's too late.

That's my layman's guess, but what do I know. All that matters is it's a sad situation and I hope the parents find closure someday.
