Mixed media filters are super common.

They Use GAC, Granular Activated Carbon, Silica Sand (various types and grades), pea gravel and periodically back wash them regenerate the filter media.

The BAC surface is cleaned/scrubbed by the backwashing.

I have Used BAGAC for 25 years, Biologically Activated GAC, where the bacteria regenerate the Carbon in a cyclic process. The carbon catches periodic high concentrations of Phenol, Benzene, etc and the bacteria help desorb-regenerate the carbon as the pollutants slowly desorb from GAC-when the pollutants are not present in the water flow. Diffusion drives it, and we pump O2 into water feeding the GAC-carbon/microbes that attach to the carbon in a biofilm.

Back washing strips excess biomass. One of the more really cool processes I set up many moons ago. It had never been done before in the application where I did it. At the time, It had only recently been tried in PhD thesis.
