Mistakes happen because players aren't focused.

Not being focused means you are GIVING AWAY what your other team mates have worked for.

Stupid mistakes.

Other factors ............. Why do holds happen?

They are getting beat their ego can't handle it so they hold committing penalties giving the game away if the other team capitalizes on the penalties.
