Reading the preliminary reports this is looking more and more like it's 100% on the operator's lack of proper maintenance and operational procedures.

When the accident first happened some airline pilots were coming out complaining how Boeing had installed this "secret" MCAS system and had not trained the pilots on it. That was walked back in subsequent days by both the airlines and the pilot's unions. The system is described in the literature, although there is no change in cockpit procedures. So obviously the pilots speaking up about the "secret system" simply hadn't been cracking the books - oops! I would imagine the written test for MAX qualification has been revised to ask a few detailed questions about MCAS? I hope??
