It is lazy, seen this often. Especially when a company redomiciles, they still just change the narrative in the 8-K and leave all the headers. But it is lazy and shoddy work by an attorney or former attorney.

If they had an auditor and sent a copy, they might have had a third or more set of eyes to catch it.
