There are other causes, certificate not physically delivered but promised by the TA. MM will allow sale based upon good faith and documents, up to 35 days, exception is granted under Rule 203 (b).

It happens, but that customer usually has a very well established account and the MM has good communication with the TA on the physical delivery.
