I have no idea who FDA's new AZ proposal would work in practice - how do you identify those at risk for developing it so you can enroll them in trials? How can you prove a drug slowed down progression when individuals all have different rates? I mean it can take years just to go from stage 1 to stage 2 - the process can be so slow I don't know how you prove a drug did anything to help - and since some can survive up to 20 yrs, do you really expect people to stay in drug trials for a decade?
