There are 5 patients. All die. They plot like so:

month 24 - 1 dies
month 25 - 1 dies
month 26 - 1 dies
month 27 - 1 dies

Median OS is 26 months

BUT, if there are 5 patients, 3 die and they plot like this:

month 24 - 1 dies
month 26 - 1 dies
month 28 - 1 dies

The trial is unblinded. 2 patients are still alive and are censored...

both (2) at month 25

So the plot looks like this:

month 24 - 1 dies
month 25 - 2 censored
month 26 - 1 dies
month 28 - 1 dies

What is the median?

The median in both cases is 26 months. Not the most illustrative example.

wouldn't really contribute to an actual OS median (if that is what was being measured - as it is in this trial),

median survival (or median progression free survival) is/are not the endpoints of this trial.

it is simply survival, as compared between the two arms, and tested using the log rank test and described by the HR.
