I thought this was interesting.

“Inflammation” as a general category was defined by the presence of inflammatory cells, including lymphocytes, macrophage, neutrophils or their combination. Lymphocytes were identified as small round cells with scant cytoplasm and darkly staining nuclei. Lymphocytes and all other histopathologic features were categorized as absent (0), present (1+) or abundant (2+) by two neuropathologists (MC, KA, RM, NL, RM, MJS, DJB) and adjudicated by a third. Cases with a complete absence of lymphocytes were labeled 0 (absent). Cases with lymphocytes in <50% of tumor tissue were categorized 1+ (present), while cases with lymphocytes in ≥50% were categorized 2+ (abundant) (Figure 1). -- www.ncbi.nlm.nih.gov/pmc/articles/PMC3865611/



This chart, link here, which I already posted previously, and the comment above might help us further understand pseudo progression gradations -- possibly -- I'm not certain.
