Koman, it's certainly your right to disagree, and I won't argue that firms have had to pay fines in the past for failing to file the proper forms. However, your only question is whether Woodford failed, and there's nothing to support a contention that he has.

Looked a little further (see the Soros filings attached), and while he files mostly 13's, there are also some 4's. In looking at them in more detail, it appears that the former are filed when only legal entities (corporation, partnership, etc.)are involved, and the 4's when individuals are also named in addition to the entities.
