AOT 'officially' started operating June 2014?

Well, that's what the 10K says and that's what the revenue statement says.

I admit to not understanding this, since it was clearly attached to the line before then. The 10K from earlier this year also said that "the equipment was delivered to the installation site in March 2014 and installation was completed and accepted by TransCanada in April 2014." Perhaps it was simply not turned on, or calibrated, or maybe STWA and TransCanada were just running tests and so forth.

In any case, given that June is the month agreed upon, it seems even more extraordinary to me that TransCanada would within a month or two of operation send a notice to terminate the lease, and that they were working to expedite testing. That reinforces the binary proposition: either it was a massive failure and they saw that right away, or it was a raging success.
