You need to tell them that you take it for CV risk reduction and V has an FDA-approved CV label but GV does not.
Do not get suckered in to discussing TG levels. Once that occurs, you lose. Focus on CV risk reduction and FDA labels.
Tell them that "failing on GV" means a stroke, MI or CV death, so that isn't an option.
If you're doing this orally, ask them if they're advising you to go against FDA guidance - this should end the conversation.
FYI I appealed orally only for my mom (different insurance) and the conversation ended after stated the aforementioned.
My BCBS-FEP formulary exemption form (filled out by doctor but I gave him a ton of info) was approved right away. I will get my hands on that form and will let you know how it was filled out.

Got V back on the Formulary. Slightly more expensive than before ($97 for 3 months), but I'll certainly take it. The Generic would cost $40 for 3 months. Clearly a no-brainer for V.
Oh, and Amarin did absolutely nothing to help even after I called them.

Big update for BCBS-FEP folks.
My formulary exemption was approved and this is good for 2 years.
BCBS-FEP's contract price for 3 months is $911. Because it is a non-preferred drug, I am responsible for 60% of that amount, which brings it down to $547. After applying Amarin's savings card ($150 × 3), it brings the price down to $97 for 3 months ($547 - $450).
So if your formulary exemption gets approved by BCBS-FEP, you will pay $97 for 3 months.
Which tier is it in and can the tier be reduced? This is convoluted. Although it shows up as tier 2 on BCBS-FEP's end, it gives the pricing of a tier 3 drug.
Tier 2 = patient pays $40 copayment; would have been $9 for 3 months after Amarin's savings card, as it has been until 2023.
Tier 3 = patient pays 60% of the contracted price.
Why does it give tier 3 pricing if it shows up as a tier 2 drug? The answer was very convoluted and I don't think that BCBS-FEP reps fully understand it but it has something to do with it being non preferred. Not complaining but wanted to fully understand it.
It is not eligible for a tier reduction - I may make one more phone call and pick their brains further.

But that's a catch-22. Is the US market dead BECAUSE they refuse to pursue legal challenges?
If they can show to the court that by placing Generics in a lower tier, induces infringement, insurance companies would have to place Vascepa in the same tier as Generics; the USA market would be regained.

As I understand, it's a 2 step fight.
1. Formulary exemption.
2. If that's approved, it'll be at tier 3 pricing (you pay 60%). You can then ask them to reduce tier/price, but that will be a difficult ask, so I'm told.

After obtaining the control number, I went to the website that Sarissa instructed us to go to and voted "for" in all 9 questions, opposite of all "board recommendations".
I didn't receive any cards in mail nor in my TD Ameritrade account and I didn't see any mention of blue or white on that website as I voted.
If I understand correctly, the blue/white cards are simply meant to dumb it down for the people who aren't as knowledgeable as we are. Double negatives can confuse some folks.

I did not receive anything from TD Ameritrade, so I called them to obtain a control number. The customer service representative called someone to obtain the control number and provided it to me within 5-10 minutes. He said that these will be mailed and become available on the TD Ameritrade website soon but he doesn't know when. So instead of waiting for that, I went to www.proxyvote.com & entered my control number & voted.
