The hepatic portal system is designed to rid the body of toxins, and it cannot detect those that are designed to help it. Some drugs must be taken under the tongue, through the skin, or via suppository to avoid entering the hepatic portal system and being prematurely metabolized in the liver before reaching general circulation.

In my days in hospital pharmacy in the IV room,Dr's orders for IV's would read for example:1000ml D5W with 200mg methotrexate (cancer drug).
As such,I would take a 1 liter bottle of D5W and a 10ml vial of methotrexate (50mg/cc),draw out 4cc and inject into the IV solution.
The IV solution is the "carrier" for the strength (dosage) of medication the Dr wants the patient to have .
