I believe that one day when the vast majority of abdominal surgeries are done through one small incision with relative ease and accuracy with robotics, ventral hernias will be rare occurrences making the ventral hernia repair argument a mute point.

I've not used fibrin glue. Is it expensive? It would be fast. Yes, tacks have complications and can cause increased pain. I prefer absorbable tacks but sutures or fibrin glue (if it works) would be safest.
There are different opinions on this. The tacks can hurt and cost much more than suture. Closing the fascia with suture from inside the abdomen than reinforcing with mesh using sutures is a more natural repair and better to me. No studies to back it up. To new. Aesthetics inside abdomen have nothing to do with the repair quality. Tactile feedback not needed. Time will tell. JMHO.

I will talk to CPA try to QEF designation for my taxable account as well but would this even matter in an IRA or 401k which you don't pay any capital gains? Why did you switch from the 401k to the the taxable account?

You can only rollover a 401k to an IRA if you change employers (or above a certain age I think). That's what I did when changed jobs last year. I used a big chunk to buy Titan, hence the sick feeling I got with the drop. Long Titan!

I know. I was always jealous until I tried robotics. I do vascular access as well so I do get to play with some toys their.
General surgery is changing and reps will be appreciated more with time. I used to scoff at reps and avoid them like the plague. Then I realized they are the the bridge to advancing technology. That light went off when I realized that well trained orthopedic and spine surgeons totally rely on reps. I hope it doesn't get to that point with general surgery but it will be close. IMO.
I agree with most of what you said but I would gladly work with a former excellent ISRG rep if they work for Titan. We would both have a common enemy. Direct quote from a recent resignation in a text to me when I asked why she left the company......."this just wasn't the right company for me. Loved my job and my surgeons but I should be respected by who I work for and wasn't."
