That's a familiar name. Sometimes you'll see her go with just Amy S Chaffe or Amy Swanson or Amy Munroe Swanson. Not sure why she likes to use so many different variations of her name in public filings. Maybe she thinks people are stupid and it will confuse them.

Amy is the long time girlfriend of Michael J Forster and had been fronting for him for a long time. They were supposed to be getting married in April of 2016 but I see the link for the wedding has been changed to no longer include Forster so I'm not sure if they actually did get married or not.
