Crow3, CDEX endorsements come at times from those that know first hand what those medication errors can really mean in human terms. Interestingly, in this particular case, I wonder if the earlier generation ValiMed systems that used only fluorescence would have included the signatures required to have helped here. However, I strongly suspect the ValiMed G4 system, with its implementation of Raman, Fluorescence, and Reflective processing, could possibly/reasonably contain signatures to identify errors similar to this. In any event, I truly hope the evaluations by hospitals include the scenario that resulted in this event.
