I like the analogy of break and accelerator , however I still wonder why somebody will use Bavi as accelerator when other tumor specific more potent MAB are available / under development.


The biggest problem with Bavi is that ,it can not work as single agent and I have hard time to imagine that somebody will try to mix PD1 + Bavi + Chemotherapy ........as it may cause adverse effect on patient.

Maybe you can help me out here!

Bavi works because a side effect of a tumor and tumor blood vessels is that they get damaged cells and PS comes to the surface. This PS activates receptors on T-cells and from there has an immune system suppressing effect (the tumor is not massively attacked because the troops are not called in).

Bavi, by binding with PS, disables PS from achieving this suppression and therefor the immune system starts doing its work, it attacks.

With chemo and radio the goal is to damage the cancer cells so that they can no longer proliferate (cell division) the cancerous cells that hold damaged DNA, the source of the cancer. The presence of Bavi keeps all the extra exposed PS from suppressing the immune system.

If we combine PD-1+Bavi (and no chemo/radio) then Bavi can only act on PS from damaged cells of the tumor and blood vessels. Till there no difference with Bavi without chemo/radio except that in absence of these therapies tumor cells are no longer destroy to keep them from proliferating. That is why Bavi alone doesn't work as far as we know because the tumor cells that are not yet damaged don't get attacked and keep the tumor growing.

As i understand it that is where PD-1 will come in. PD-1 will remove the break from the T-cells by interacting with receptors which will result in these T-cells to attack the tumor cells (targeted as I understood although that part isn't clear yet to me). Those tumor cells can then no longer proliferate and so PD-1 fulfilled the function that previously chemo/radio did.

However PD-1 cannot do that effectively if the PS that is already around due to damaged tumor and blood vessel cells suppresses the T-cells again from calling in the troops. Actually PS oppose the good working of PD-1 and could even be the cause that they can never fully remove the tumor, always being a little late and giving the tumor a change to grow some more.

So PD-1 removes a break and PS pulls the emergency break while in combo Bavi can keep PS from pulling the emergency break.

Therefore a cocktail of both works better and requires no more chemo or radio. Since as a consequence this will result in less PS exposure from the chemo/radio damage, Bavi will be able to work more efficiently and probably won't need 3mg/kg body weight because Bavi only needs to disable the primary PS that keeps the PD-1 from being efficient and not massive amounts secondary PS from chemo/radio cell damage.


It also means the immune system will be activated and the tumor will eventually disappear. No more need to keep sending fireman (another of those great semaphores of this board). No side effects from the chemo/radio and therefor more time to treat the patients and more survivals. Furthermore Bavi is safe and as far as we know PD-1 are save too, certainly compared to the other damaging therapies.

This sounds like low invasive treatment of solid cancers and has something like the beginning of a possible cure.

And since PPHM is the only company that has such substance that can suppress PS it sounds like we are indeed another horse in our stable. We can now play in many scenario's. The traditional chemo/radio/cotara+BAVI, the PD-1+Bavi and the Full Human Bavi (imaging) and the Beta Body paths exploration.

From a BP perspective, the one combining his chemo/radio or PD-1 with Bavi will have better chances to become the new SOC.

If the PD-1's don't make it and remain just an ASCO 2013 hype then we are in all cases a play in the chemo/radio arena too and there also have an upcoming horse called cotara for solid cancers.

FF I noticed they repeated the "checkpoint" language on the new FACTSHEET. As it is just below the visual which shows some of the complexity of the immune system activation perhaps it is still a useful term ? But I understand FTM's objection as it does have a narrower technical usuage.
