Hospitals and transportation companies have those insurances NOT per substance but per activity base.
PPHM needs one for all their own manipulations (could be Avid). They have special containers for the transport and for light medical and electronics grade amounts the insurance isn't that high.
However, for radio therapy and so it is different.

In the military spark-gaps (closed and filled with a radio active substance to lower the needed Paschen current for discharge and accelerate the Townsend avalanche is a very common application of radio active substance applied in electronics.
