To his credit, Jack did point out it is an entirely new compound, but missed the opportunity to state Q is a combo of two existing compounds one of which causes birth defects and, therefore, should not be used by women of childbearing age. Doctors are risk averse and, like most of us, vote with their wallets. And Eisai has a great distribution network that already markets 14 other drugs.
