Although I generally agree that insurers think twice before interfering with decision making in a protected category, one consideration that is relatively unique to HIV is the large number of patients whose treatment is funded by the government or public assistance programs (Ryan White, ADAP, prison budgets, etc.) - that business will likely be subject to step edits as soon as Truvada goes generic.
