One problem with that Q&A session. You asked if there was any additional fees associated with "placing the trade" and this does not address any fees imposed by the clearing house that processes the trade which is where NOOB posted that the $200-$700 fees come from.
I have contacted Etrade about these types of trades in the past. Some brokers clear their own trades therefore no need to go through another clearing house such as Penson Worldwide. Any companies that do not clear there own have to pass on these extremely high fees which again is where the $200-$700 charge was posted as coming from.
Be careful accepting the terms before you are clear on what they are! I have heard of people getting slammed with these high fees MONTHS after the trades were done. Ask your specific brokers what you will be charged. Trade cost + commissions and is there anything else? Or you may not like the surprise.
