That is a hard question -- We can break it down to two questions:

#1. Can I tell the difference between ad-insertion ads and national/local feed ads?

#2. Are the ad placements relating to the content viewed on TV?

When i'm watching sports, they are generally focused on sports. Evening news are usually accompanied with Viagra ads, cars or something. This is generally the same across all types of broadcast medium (DirecTV, Comcast, Cox, Time Warner, etc).

There were local ads but I couldn't tell if it is from the local feed (i.e. WCAU for NBC and WTFX for FOX) or the ad insertion technology.

Basically I can't tell if ad-insertion ads were playing or not. Generally, the ads were focused on the content playing though. However, if it is indeed working, then kudos to them because I believe it should be seamless!!!!!

For the most part, ad insertion tech was unnoticeable besides a few noticeable switches from the national feed to the local feed. I am not sure if it was TIVU to blame or that local station. I grew up around the area and in the past, they weren't the cleanest in switching to and from the national feed.

Unless you are aware of which ads are being inserted you can't "see" it happening. It's a seemless transition like any other commercial.
