I can't ascertain from the questions how significant items 2, 6 & 10 are.

I am an engineer by formal education. Great engineering dissasters are not the result of faulty engineering. They are the result of faulty assumptions made by the engineers.

It looks like there were some pretty basic contradictions about the tailings stuff. That's a lot more than fluff.
