The company was issued 2 permits by the NYS DEC on 6/14/11. The items in my post were taken from the AIR STATE FACILITY PERMIT:

They were also issued a SOLID WASTE MANAGEMENT PERMIT:

The above links were taken from the 8-K filed by the company....I was unable to find links on the Plastic2oil.com website. If you need to search through the Air permit, I found this pdf version easier to use:
