This would still result in wastage of 45-IU (your former "25% of the purchased drug being wasted" quiz), which I assume is difficult to avoid as it isn't likely that docs can schedule patients in a row very often.

Good job in connecting the two quizzes (which everyone else missed). For high-volume docs, scheduling patients in contiguous slots ought to be doable; for run-of-the-mill injectors, however, the 45 IU will be lost and migraine patients of these docs will presumably be charged a higher price.
