Ask for Eric in corporate actions. From the 6-7 people I have talked to he has definatly been most helpful. He's knowledgable about the matter and will get whatever you need straightened out.

make sure you contact Abby at Island Stock Transfer and let her know your shares are coming and to hold them if they arrive via physical certificate form

Also send her the Stock Power, SDVI conversion document, and $25 check

I spent two weeks in limbo because etrade sent my shares via physcial certificate to island, but island didn't have those forms so they sent them back to the Depository Trust Corporation which I had to have etrade then pull them back out. I ended up having etrade send the physical certificates directly to my house and I then mailed them to Graffiti myself

Etrade has messed up alot of the DRS actions. They have sent around 4 of my friends via physical cert instead of electronic DRS. Make sure you make sure the rep knows its DRS eligible and can be found as "signaturedevices" with no space. The one rep I talked to found it that way as it didn't show up via "SDVI" or "Signature Devices"
