I dont work for a T/A or a Broker so I dont know the exact process, I just know that My broker told me that company will give the T/A my information that was on the form and the T/A will contact my broker. I called multiple times and got the same answer everytime and 2 of those times the person on the phone said hold on let me verify this with my manager.
