There are several mysteries to me in that request.

The O/S and Float is one, and what the hell are common restricted shares. I thought those were for a 504 offering.

It just made me say Hmmmm...

nukuhiva, re. the importance of 10% ownership of ONCP.
Insiders are defined as management and those holding 10% or more of the OS. Insiders must make certain SEC disclosure filings many investors had rather not fool with.
It's fairly obvious "(1) a." within the Conversion Request form is an attempt to help those who might not want to assume 10% or more oncp ownership.
