Matthew - I recommend you DELETE the section which jackface2 mentioned in his posting to you (referenced below), and ONLY list 2 items:

Authorized Shares: [Number] as of [Date]
Outstanding Shares: [Number] as of [Date]

The other information is so outdated as to be worthless at best if not unintentionally misleading...
