I've been trying to find out the answers to your questions on the net, but haven't been able to find any definite answers yet. It looks like most companies get a lump sum upfront along with a percentage, but I'm sure each deal is different and I'm not sure how we could find out specifics (seeing how most of that would be covered in their contract with each company and I don't see us getting access to their contracts)...
