I believe you are only guaranteed a fill on the OTC if your buy order is at ask. My situation was different. I had a sell order at $1.63 when the spread was $1.63 X $1.65. My order was never executed and the next thing I knew the stock was trading at $1.50.

Not too happy, I called TDAmeritrade who in turn called the MM. Within a few minutes there was an execution in my account at $1.63.
