Just look at all the clerks populating your doctor's office, there to check your insurance, put out more forms warning you that if the insurer won't pay you're on the hook, do all the back and forth on payments and collect your co-pay. In a single-pay system you don't have any of that waste.

We were just in Turkey where my son sprained his wrist. We took him to the ER at the nearest hospital. The only form info they needed before X-raying him was his first and last name! No insurance info, no credit card, nothing. The whole thing from arrival through X-ray, medic checking the X-ray, collecting a bandage and a PA type bandaging him up took well less than an hour and cost $25, and we had a wheelchair attendant and gal who served as translator with us for the entire time. I wouldn't necessarily go back there for heart surgery, but when you eliminate the wallet biopsy from the system the experience is a whole lot pleasanter, more trusting, and cheaper.
