I just spoke with Theresa about the certs. I had questions and she answered my voicemail within an hour of me calling. Simply download the Stock Power form and the Application. Bring the Stock Power form into your bank (most banks have a New York "presence"). If yours does not then your bank will be able to guide you to another bank that will do it for you. I'm with Bank of America so there should be no issue. You'll need them to Medallion Stamp your Stock Power form and have the rep. from the bank sign it. You'll need to sign it also and give the necessary info on that form. Fill out the application, self-explanatory. You don't need to anything to your certificate. After filling in the necessary 2 forms send certified mail to the corp. address given with the instructions. You'll be mailing 1 or more certificates, 1 page Stock Power, and 1 page Application.
