In a nutshell, both depending on how long and at what temperature it has been kept. At the very least, it makes the vaccines totally ineffective.

In my experience (hospital pharmacist), storage reccs are made based off loss of active ingredient in excess of 10%. Therefore, if they say meds can be kept out of freezer for x hours. You would expect a loss of > 10 % active ingredient after x hrs.
