No one should lie on the form, because there will be something saying that you certify that all information is true.

They will probably compare the info received with the shareholders list which they have probably already obtained. At least that's how it was done for Chemtura Corp. (CEMJQ) when we shareholders had to provide our info to the Equity Committee.
Anyone trying to pull a fast one would be discovered.
