He lists two "Entities in which you are an owner or officer" (in addition to Nulixo, Inc.) in the current year....(1)LATERAL PROTECTION PROGRAM, LLC and (2)ALEXANDERS DIGITAL PRINT, Inc..
Both of those entities are shown by the State of Utah to have an Expired status:
(1)Status: Expired as of 12/28/2016
Status Description: Failure to File Renewal
(2)Status: Expired as of 08/22/2001
Status Description: Failure to File Renewal

It's hard to understand how one could have a conflict of interest or a need for financial disclosure based on a relationship with two expired entities and one that doesn't seem to have done anything yet.

There was an entity named Nulixo registered in Utah on 11/8/18 but contrary to both the 8-K filing and the above form it was registered as an LLC, not as a Corporation.
