Seems the issue--Urban Barns Foods Inc--was suspended in May for failing to file required SEC periodic reports--and emerged 10 days later on the grey sheets:

just a WAG as I know nothing about this company--perhaps they filed to terminate their registration/filing obligations before the SEC permanently revoked them and permanently killed them--which is certainly where they were headed...
